Promoting Partnerships-Achieving Positive Outcomes for
Students with Chronic Health Conditions Conference

The purpose of the Promoting Partnerships Conference is 3-fold:
- To increase school personnel awareness of the academic and social needs of students with chronic health conditions;
- To improve access throughout the school day for students with chronic health conditions; and
- To assist school teams to develop individual plans for students with chronic health conditions.
Persons most likely to be interested in this conference include:
Students with chronic health conditions; parents; school nurses; classroom teachers; special education staff; guidance counselors; related school health professionals; primary care coordinators; family practices; PIH Family Support Coordinators; Special Medical Services coordinators; and pediatricians.
School teams:
Each school team includes an administrator, teacher, parent, school nurse, and special educator.
Purpose:
To focus on learning about and planning for your students with chronic health conditions.
Note:
Morning Session from 9:00 AM to 12:15 PM, including lunch, is open to all conference registrants.Afternoon session is dedicated to School Teams and the Focus Group..
