Welcome to the New Hampshire Council for Children and Adolescents with Chronic Health Conditions Web Site!

The Council for Children and Adolescents with Chronic Health Conditions is established to advocate and collaborate with state and local agencies to assess and improve:

Access to regular school and health care,
Capacities of communities to effectively respond to needs of children and adolescents with chronic health conditions, and
The delivery of meaningful services and support to children and adolescents with chronic health conditions and their families.

These children and their families face numerous challenges, including school and education issues, insurance and medical concerns, and family support needs. All too often children with chronic health conditions do not receive the services, support, and attention that they critically need. Initiatives of the Council include: Health Education Leadership Project (HELP); Guaranteed Collateral Loan Fund, Parent Training/Conference Funds; School Personnel Training/Conference Funds; and Family Financial Management Workshops.
